we want to import the data, one of our filed is check box,
how to create or what is the format or structure of the filed chexbox adn its headings

Second thing, I advise you to begin by reading manuals and tutorials
Now, for the question above, if you have a look in database, you will see that checkbox are stored as a string, and each value is separated by comma.
